Responsibilities:
Under the general supervision of the department director, reporting to the Social Work Supervisor, this position serves as a member of an integrated, multidisciplinary team that is focused on the assessment, support and/or post-acute disposition of NWH patients and their families. This position will seek opportunities to create improvements in resource utilization, length of stay and appropriate discharge planning criteria. These duties will be performed in accordance with, and in adherence to, the mission, vision, and values of Northwest Hospital.

Qualifications:
Education Background :
Master of Social Work Degree from a program accredited by the Council of Social Work Education Eligible to be a practicum instructor for the University of Washington School of Social Work preferred.

Licensur! e, Registration, Certification

Credentialed as any of the foll! owing for all applicants after June 1, 2013:
Licensed Social Work Associate Advanced (LSWAA),
Licensed Social Work Associate Independent Clinical (LSWAIC),
Licensed Advanced Social Worker (LASW),
Licensed Social Work Independent Clinical (LICSW)

Work Experience
One year full time professional social work experience including discharge planning and resource utilization in a health-care setting or equivalent required. Two plus years preferred.

Skills
Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit/department.
Ability to flex to assist other department members as needed.
Psychiatric assessment skills at a level consistent with social work job assignment
Ability to handle a fluctuating workload
Ability to communicate clearly in English (verbal and written) with all stakeholders (staff members, clinicians, practitioners and the genera! l public.
Computer skills consistent with electronic charting and referral system

Physical Demand Level: Sedentary
In an 8-hour workday, must be able to-
Lift / Carry 10 pounds occasionally
As required, must be able to -
Sit, Stand, Walk, Bend, Squat, Kneel, Crawl, Climb and Reach above shoulders
Push and Pull
Perform fine motor functions
Must possess functional vision, hearing, and speech in order to communicate effectively with clients, client advocates, and staff.
