RN case manager/Public Health Nurse

Provide patient care services within Snoqualmie Health Clinics including triage and ambulatory care nursing services. Case management for elder care services, diabetes, cancer and other complex care conditions, including women's health, pain management, mental or behavioral health. Provide potential and/or shared supervision for licensed and unlicensed clinical support staff. Quality assurance for direct and indirect patient care services within Snoqualmie Health Services.

DUTIES

Must be able to provide all aspects of quality patient ca! re in an ambulatory/clinical setting.
Provide indirect pat! ient care including documentation via electronic and paper charting, fax, assessment-associated paperwork, referrals, telephone messages and other correspondence.
Provide complete, accurate and timely documentation of patient encounters, phone records and case reports.
Collaborate with case management team and provide nursing care for Snoqualmie Tribal clients, Diabetes and
Immunization programs, and other complex conditions such as cancer, mental/behavioral health and women's health.
Assist with clinic supply/equipment orders and inventory maintenance.
Prepare medical emergency supplies, restock as necessary.
Review current protocols for emergency response and ensure staff are trained and prepared to respond.
Coordinate and schedule patient visits to adequately meet clinic needs based on staffing and triage situations.
Work with Medical Director and nursing supervisor on program planning, protocol and policy development.
Supervise unlice! nsed clinic support staff, including training and mentoring, and assist with disciplinary action, competency assessments and performance reviews.
Assist with ambulatory care triage and telephone triage as needed.
Effectively address patient and staff complaints with problem resolution.
Ensure effective reception services are provided in a timely and professional manner, assist as needed.
Provide vaccines for Snoqualmie Casino employee vaccination program per protocol and manage adverse reaction response as indicated, maintain accurate and up-to-date vaccine records.
Program planning, policy development, Medicaid outreach and referral and/or interagency coordination related to Medicaid and Non Medicaid covered services.
Promote and provide information and referral to clients related to smoking cessation and problem gambling to assist them in leading a healthier lifestyle free from the effects of smoking and addiction.
Ensure a safe clinic environm! ent for staff and patients, complete incident reports in a timely manne! r.
Participate in clinic staff meetings, provider meetings and other workgroups as assigned.
Participate in tribal and community events as a clinic representative as assigned.
Successfully complete any necessary training, including BLS, ACLS, and PALS.
Various other clinic duties to be performed as assigned.

Education
2-4 Year Nursing Program leading to RN licensure.
4 Year BSN or Public Health Nursing education preferred.
Experience
Minimum 2 years related experience in ambulatory care or home health nursing to include a variety of complex care conditions with case management and family practice required.
Minimum 3 years inpatient nursing or public health nursing experience required.
Tribal and/or IHS healthcare experience in an ambulatory setting prefe! rred.
