This job is responsible for direct! ing and coordinating members of the care team, focusing on the provision of individualized quality patient care consistent with organizational standards. Develops the patient plan of care in partnership with physicians, interdisciplinary teams, and patient/family. This position is responsible for the coordination of care for Hospice patients who reside in the Hospice House facility. Coordination of care includes: coordination and direction of the interdisciplinary team, patient, family and facility staff; patient symptom management; and assessment of patient and family physical, psychosocial, cultural and spiritual needs. The incumbent provides end-of-life education to patient, family and facility staff and advocates for patients and families to ensure patient focused care.

Essential Duties:
Coordinates and delivers patient care by assessing, prioritizing and monitoring patient/family physical, psychosocial, cultural and spiritual needs. Communicates and! coordinates care with the Interdisciplinary Team (IDT), patie! nt caregivers and facility staff. Maintains communication with patient's physician, caregivers, facility staff, and Hospice team members. Advocates for patients and families to ensure patient focused care.

Supervises the Hospice LPN and C.N.A. and updates the C.N.A. Plan of Care to assure that the current patients' needs are reflected on the Plan of Care and communicated per policy.

Implements and revises the patient Plan of Care considering physical, psychosocial, cultural, spiritual and financial resources in conjunction with the IDT and facility staff. Modifies the Plan of Care as patient or family changes occur and communicates changes to the patient care team. Attends and participates in daily report, team meetings, briefings, in-services, department and staff meetings and other related activities.

Directs the workflow, makes out assignments, and ensures that patient care staff tasks are being performed on a day-to-day basis. Provides the cli! nical manager with performance feedback for the patient care staff he/she supervises.

Completes/communicates documentation of pertinent patient information. Assures that an updated Hospice IDT Plan of Care is in facility chart. Documents pain/symptom management interventions, telephone conversations/consultations with other caregivers or medical professionals. Updates changes to medication record. Completes transfer/discharge summaries

Provides patient, family and facility staff Hospice education.

Assesses learning readiness and provides appropriate education. Assesses patient, family and facility staff knowledge of disease process, symptom management and end-of-life care, providing education when necessary.

Evaluates and admits patients to Hospice service in conjunction with the interdisciplinary team. Keeps referral source/primary physician informed of patient's admission status. Completes admission reports and documentation. Reviews pa! tient information in the admission packet with the patient and family. !

Qualifications

Education:
Graduate of an accredited school of nursing.

Bachelor of Science in Nursing is preferred

.

Experience:
Minimum of two years experience within the last five years in an acute or subacute (i.e., nursing home) setting with a minimum of one year in the following areas: oncology, med/surg, gerontology, home health or hospice.

Supervisory or leadership experience preferred.

Experience necessary in interacting with an interdisciplinary clinical team and physicians preferred.

License/Certification:
Current State Licensure as a Registered Nurse required.

Current CPR certification

Active membership or certification in relevant professional associations preferred.

Hospice and Palliative Care (CRNH) certification is preferred.

BLS certification required.
