Primary Purpose :
Under the supervision of the Director of N! ursing Services, oversees the delivery of medication and nursing services for the residents, ensuring that staffing, supplies, and services are provided at the highest level and meet all requirements. Located in the Health Center, oversees the daily operations of the nursing services.

Principle Responsibilities :

  • Reviews staff schedule and makes arrangements to cover for any absent staff.
  • Reviews routines, calendar of events, and resident needs, delegates and assigns staff to work accordingly. Conducts shift change report meetings.
  • Coordinates arrangements for resident appointments, treatments, or procedures, including transportation. Ensures that ‘vitals’, histories, and physicals are done as scheduled.
  • Processes physician orders.
  • Makes daily rounds to observe and evaluate physical and emotional status of residents.
  • Coordinates admission and discharge of residents.
  • Supervises and observe! s staff in the performance and their duties; provides them wit! h feedback and any needed guidance.
  • Assists in training new staff and documents performance of all nursing staff. Meets with staff to ascertain their needs and suggestions.
  • Reviews and updates resident care plans for problems, needs, appropriate goals, and approaches.
  • Reviews medication profiles for completeness of information, accuracy in physician order transcription, and adheres to stop condition changes. Assists in ensuring that medication is provided in accordance with State law.
  • Ensures that medications and supplies are ordered and in stock. Ensures that scheduled drugs are counted at shift change.
  • Keeps the Director of Nursing informed of residents’ status and related matters.
  • Communicates with Administration, Social Services, and Chaplain as necessary.
  • Provides direct resident care, treatment, and medication as ordered. Charts on residents as necessary and required. Assures that all charting and record! keeping is completed.
  • Updates families as to resident condition â€" in accordance with policies and procedures.
  • Follows up on dietary requirements and recommendations.
  • Participates in Quarterly Care Plan reviews.
  • Updates the medical and NAC Flow Sheets and order sheets at end of month.
  • Communicates with physicians as required. Keeps medication sheets current and updates Phys. Orders.
  • Participates in investigating, documenting, and resolving complaints by residents and family members.
  • Performs administrative duties such as telephone work, faxes, filing, records management, scheduling, charting, and paperwork.
  • Attends meetings, in-services, and participates in fire and disaster drills.
  • Assists DNS as required.
  • Coordinates and communicates with residents’ physician, family, other professionals, and caregivers the ordered care for each resident.
  • Performs assessments on resident! s, provides information for completion of nursing components of MDS. Pe! rforms quarterly reviews and assessments of assigned residents per policy and procedure.
  • Monitors residents for mental, physical and/or emotional changes. Reports any changes to appropriate staff.
  • Performs nursing functions in a professional manner to the extent and within the limits of the scope of licensure with the State of Washington.
  • Institutes alert charting as needed per procedure.
  • In concert with other registered nurse, disposes of discontinued or outdated medications according to pharmacy policy and procedure in a timely manner. Records disposition on appropriate form and places form in resident chart.
  • Prepares appropriate documents in relationship to sending residents to another facility, hospital or when ordering lab and x-ray.
  • Directs NACs nursing care according to basic standard of care and nursing directives/care plans. Gives report and returns report with NACs on each shift.
  • Makes walking rounds w! ith NACs while doing report, checking rooms and residents’ care each shift.
  • Reinforces to NACs importance of Knock-Name-Wait-Wash-Pull and Explain when going into a resident’s room.
  • Participates in performance evaluations of nursing assistants. Communicates concerns about NAC performance to Director of Nursing.
  • Confers regularly with the DNS on resident conditions, unusual circumstances, admissions, and incidents.
  • Completes incident reports as needed and conducts the initial investigation. Summarizes and reaches a conclusion to the investigation as possible.
  • Helps ensure Health Center, assisted living and clinic are clean and odor free.
  • Demonstrates a compassionate and caring relationship with residents, families and co-workers.
  • Enforces universal precautions, wearing gloves, gown, mask and goggles as needed.
  • Confers regularly with physician and reports changes of condition.
  • Cleans and disi! nfects cart surface as needed. Cleans medication storage area.
  • Remains aware of infection control issues and implements necessary changes.
  • Discusses with related professionals on current resident conditions and changes via formal and informal reporting to ensure appropriate and consistent care.
  • Attends routine nursing meetings & scheduled in-service trainings.
  • Responds to emergencies in independent living & assisted living as needed. Performs first aid as needed to IL & AL residents. Provides assessment & advice to independent living residents in emergencies.
  • Performs other duties as assigned.

Education and/or Experience, Required Skills/Knowledge/Abilities:

  • Four year college or university education (preferred)
  • Must! be a Registered Nurse in state of Washington with a minimum of five years related experience, with experience in Geriatrics preferred. May also consider LPNs.
  • Must be able to provide leadership and supervision over other staff.
  • Must be accurate, detail-oriented, and able to keep thorough records and charts.
  • Must have knowledge of DSHS Regulations, with good communication skills in speaking and writing.
  • Excellent interpersonal and communication skills and the ability to work with staff and external constituents.
