is calling a Home Health Registered Nurse Case Manager to Providence Hospice & Home Care of Sno! homish County in Everett, WA.
We are seeking an experienced, independent and compassionate RN to provide quality care to patients and their families in the home.
This is a 0.6 FTE (24 hours per week) position.
In this position you will:
Drive throughout your work day to see patients in private residences, assisted living, and skilled nursing facilities within a designated region
Assess, plan, implement and evaluate nursing care for the patient utilizing the interdisciplinary care team
Demonstrate critical thinking skills, decisive judgment and the ability to work with minimal supervision
Utilize software programs for the maintenance of patient records and communication with team members
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure
Current driver’s license and insured, reliable vehic! le
Strong computer keyboarding skills and experience ! working with an Electronic Medical Record (EMR) system
Minimum two (2) years nursing experience
Preferred qualifications for this position include:
Bachelor’s Degree in Nursing (BSN)
Current Washington State Registered Nurse (RN) licensure
Current Washington State driver’s license
Current CPR/BLS certification
Experience in a Medicare certified Home Health or Hospice agency
Experience with Microsoft Office software programs including Outlook, Word, and/or Excel
